I give my permission for the St. Andrew By-The-Sea United Methodist Church (Hilton Head Is., South Carolina) Scholarship Committee to have access to my records maintained at the school which I am attending for verification of scholarship selection criteria.

I understand that this scholarship application and support information becomes the property of St. Andrew By-The-Sea United Methodist Church Scholarship Committee.

I agree that St. Andrew By-The-Sea United Methodist Church Scholarship Committee shall have discretionary authority (including use for publicity purposes) in all matters pertaining to this scholarship.

I agree, if awarded a scholarship, to complete all contractual obligations including acknowledging the sponsor as stipulated.

I agree to visit St. Andrew By-The-Sea UMC during an event or function at least once per year, upon request.

I certify that the information in this application is complete and accurate to the best of my knowledge. I will notify St. Andrew By-The-Sea United Methodist Church Scholarship Committee if there are any changes.
